I got this rear disc brake conversion kit from lugnut4x4.com and it needed a bit modification to make it work luckily it was fairly simple to do. My truck is a 1991 dodge ram w250 cummins 3/4 ton dana 60 rear axle, I believe the dana 60 and 70 have the same type of bracket.

MACHINE BUSHING SPACER FOR BRACKET
I would go to a hardware store and ask if they have machine bushing size 1/2inch ID, 1inch OD 1 1/2 inch thick. the reason why I used 1 1/4 inch thick with a 1/4inch thick was because they didn't have any left for 1 1/2inch thick bushing.
