100% Definition: (although there is not an official one, this is agreed upon by the community, and the most logical definition)
- Complete all possible Quests and get them in the Journal. They are not allowed to fail, although single quest objectives are allowed to. While not all quests are possible, the most possible should be completed. The conflicting quests don't have to both be in the journal, but the journal should have the most quests possible
- Complete all unmarked Quests, those will not appear in your journal
- Grey-Out Every Single PoI
- Collect Every Single Crafting Diagram, this includes both Gear and Alchemy
- Craft Every Alchemy Item that is one-time craftable, anything that can be crafted multiple times is not required to be crafted
