This October 10, 2024, PQA Quality Forum Webinar focused on pharmacists’ work to improve blood pressure (BP) control.

The AMA MAP™ Hypertension Quality Improvement Program ("AMA MAP™ HTN QI"), a high-impact evidence-based quality improvement (QI) program, provides healthcare organizations (HCOs) a clear path to significant, sustained improvements in BP control. The AMA MAP™ HTN QI is centered on measuring BP accurately (M), acting rapidly in the initiation and optimizing pharmacotherapy (A), and partnering with patients (P). The pharmacist's role in acting rapidly to overcome therapeutic inertia and partnering with patients to support medication optimization is pivotal to the program’s success.

In this webinar, panelists detailed various pharmacist service models incorporated into the AMA MAP™ HTN QI across different Federally Qualified Medical Centers. They examined implementation strategies, challenges encountered, and successes achieved with the population health pharmacist services model and the community pharmacy practice model. The focus of these pharmacist services extends beyond medication adherence. It aims to enhance treatment initiation and intensification for better blood pressure control, ultimately improving patient outcomes.
