Biography

Tori Bahr MD, FAAP is an internal medicine and pediatric Complex Care physician at Gillette Children’s where she specializes in the coordination of patients with childhood onset medical complexity. She has a special interest in supporting patients and families through their pediatric to adult healthcare transition. Her passion for transition started when she was a PCA for a young woman with muscular dystrophy and was strengthened throughout her training at the University of Minnesota. She is energized by partnering with those struggling through transition and is committed to radically transforming how we care for patients in this important life stage. She hopes her work will inspire others across healthcare to embed transition into the care they provide every day.
